Consent forms - first instance of e-signatures during CrankWheel sessions

This release adds the ability for insurance agents and brokers in the US to have their clients fill in a CMS consent form during the session.

This is the first special case of e-signatures facilitated within a CrankWheel session. We expect to bring out more cases and likely a generic facility; we'd be interested in hearing your feedback and requests.
